Goals & Strategies
Our Goal: To prevent and reduce the burden of diabetes and CVD among the Hispanic/Latino and African American populations in Chicago , starting with five target communities: East Side, Humboldt Park , North Lawndale, South Chicago and South Lawndale .
Strategy 1: Develop effective partnerships to transform existing or introduce new policies that result in sustainable system changes that increase healthy eating and physical activity.
Strategy 2: Support CEED@Chicago members to change systems by facilitating the exchange of information among members, providing technical assistance, and disseminating information on successful practices to a wider audience.
Strategy 3: As a Center of Excellence , share our expertise and that of the CEED@Chicago Coalition, acquire additional expertise in CVD and diabetes in Hispanic/Latino and African American communities and the tools needed to combat their impact, and widely disseminate this information.
CEED@Chicago is guided by a socio-ecological model that follows community participatory and empowerment approaches. CEED@Chicago's community action plan (CAP) will be based on community engagement and mobilization. This calls for the involvement of members of the CEED@Chicago Coalition and partnership-building with multiple public and private sector agencies. Although private-sector agencies are not traditionally involved in public health policies and interventions to improve community health, as members of the CEED@Chicago Coalition, they will participate in identifying the policy changes they believe will lead to reducing CVD and diabetes.
