Community Action Plan: A Road Map for Creating Change

CEED@Chicago believes that an empowered community is vital to increase knowledge, skills, and the ability to access health resources. Therefore, the CEED@Chicago's Community Action Plan (CAP) will be developed through a true community participatory process as members of the CEED@Chicago Coalition define the approaches it will employ to reduce CVD and diabetes. The Coalition has already made the decision to work for policy changes related to health literacy and the equitable distribution of healthy food. How this will be done is another step in the writing of CEED@Chicago's CAP.

After the CEED@Chicago Coalition identified health literacy and the equitable distribution of healthy food as its two policy areas, coalition members formed committees to investigate policy change options. The two committees prepared and presented recommendations to the entire CEED@Chicago Coalition. As the Coalition makes decisions and its work takes shape, the CAP will evolve to reflect its choices.
