Our Communities
CEED@Chicago will build on its success with the Chicago REACH 2010 project which preceded CEED in East Side, North Lawndale, South Chicago and South Lawndale communities. This project improved health at the community level over the life of the program by focusing on preventing and managing diabetes and cardiovascular disease among the Hispanic/Latino and African American residents.
