The Center of Excellence in the Elimination of Disparities at Chicago (CEED@Chicago) welcomes you to learn about our goal and work.

The goal of CEED@Chicago is to prevent and reduce the burden of diabetes and CVD among the Hispanic/Latino and African American populations in the Chicago metropolitan statistical area and eventually extending to the Midwest region and the United States. CEED@Chicago is building a multi-sector Coalition composed of businesses, institutions and individuals representing local government, health professionals, academic institutions, community and professional organizations, and community leaders all dedicated to changing social determinants that are needed to reach this goal.

The CEED@Chicago Coalition identified food equity and health literacy as our two policy priority areas. Our Food Equity Policy Committee has been focusing on policies that promote the local production and distribution of healthy food as well as policies that improve available data to make it easier to identify underserved communities. CEED@Chicago’s Health Literacy/Community Health Workers Policy Committee is working to increase information about healthy eating and physical activity to our communities through the services of community health workers.

CEED@Chicago members have access to training and technical assistance. They also have opportunities to promote their work and successes.

Legacy Project Grants 2010

CEED@Chicago is pleased to announce that we are now accepting proposals for our 2010-2011 Legacy Project grants 2010. The request for proposal (RFP) is available for downloading. Click here>>

CEED@Chicago recognizes that the involvement of the broader community is critical to overall system change to improve health and eliminate social disparities. Therefore, CEED@Chicago is seeking to fund three (3) innovative community programs/projects that focus on reducing type 2 diabetes and cardiovascular disease (CVD) among African Americans and Hispanics/Latinos by promoting healthy eating and physical activity. The awards for these Legacy Projects is dependent on CDC funding to CEED@Chicago, but are expected to range from $15,000 to $20,000 for a 12-month period. The deadline for submission is 5:00 PM on Monday, August 16, 2010.

Applicants must represent an active coalition or partnership of three or more local organizations with a history of working together for at least one year. The lead organization/applicant must have IRS 501(c) 3 status and demonstrate a capacity to administer Legacy Project funds. At least one organization of the coalition must be located in the community being served by the proposed project. Projects must also demonstrate significant involvement of stakeholders in planning, implementing, and monitoring of project activities. A Legacy Project Proposal Writing Workshop will be held on July 16, 2010, from 1 - 4 PM. One hour will be devoted to questions and answers.
